• Distinguish advance care planning (ACP) from shared decision-making; 
• Discuss guidance in West Virginia law for decision-making for patients who lack decision-making capacity (DMC);
• Clarify the differences between advance directives and medical orders (DNR and POST);
• Identify recent changes to the West Virginia POST form;
• Describe where and how to document ACP forms Review what makes forms valid, state-to-state considerations, and notary/witness requirements; and
• Apply West Virginia Health Care Surrogate Law to a challenging case
